How to Roll Money into the Plan

You can transfer money from a former retirement plan or IRA into the A Place for Mom 401(k) Plan. You will need to contact the custodian where your money is currently held to initiate the rollover. You should instruct the payment as follows:

MAKE ROLLOVER CHECK PAYABLE TO:
Charles Schwab Bank; fbo: A Place for Mom, Inc. 401(k) Plan; Acct# 704705

WRITE IN THE MEMO LINE OF THE ROLLOVER CHECK:
Acct #  704705  fbo:   Participant Name and SSN

MAIL THE ROLLOVER CHECK TO:

OVERNIGHT ADDRESS:

If the custodian requires a certification document, you can download one here.

If you are new to the plan, be sure to login to your account and setup your investment elections. This will determine how the rollover funds are invested once they are deposited into the plan.
